Services included either – 2 exams; 2 or 4 cleanings (4x for Periodontal patients); 2 oral Cancer Screening; 2 Fluoride Treatments; 1 set of 4 Digital Bitewing X-rays (if needed); 1 Periodontal Screening; 1 Emergency Exam/Visit (used any time during the year)
Payment for above services is due at the time care is rendered. * Services in excess of $1000 will require a deposit
Important Plan Details
Coverage is in effect for one year with signup date serving as the plan anniversary. Membership fees are non-refundable. Appointment times may be limited. It is the responsibility of the member to schedule appropriate visits during membership period. No refund of unused appointments (no rollover) Membership is available to all non-insured patients. Excludes care covered under workman’s comp or auto coverage. Savings valid at Your Syracuse Family Dentist only. Services requiring the care of a Specialist are not covered by this plan.
